After the document is downloaded to your computer, open the document and click on the line (_) at the end of each question and type your answer.  The response will automatically underline as you type and will do a Return to continue the paragraph.

Please note: We only offer financial assistance to programs that are GARR, CARF, THOR or Joint Commission approved.

If financial assistance is awarded, it will cover weekly program fees, but we do not cover admission fees. If there is a particular program the applicant is interested in, it must be noted on the application as well.

A minimum of $100 must be paid by the applicant to the program they are being admitted to in order to qualify for a scholarship.

INCOMPLETE APPLICATIONS WILL BE RETURNED AND WILL NOT BE CONSIDERED FOR FUNDING UNTIL ALL COMPONENTS ARE COMPLETE, INCLUDING A FORM OF ID
